I want to add columns to the submission spreadsheet that the form feeds in to. I am not editing any of the existing columns, just adding more columns at the end. I did this one time and they stayed, but it seems that after a new person submits a form, my custom columns were deleted. Please advise.

You can actually insert column to the right most end of the integrated spreadsheet and still retain the submissions.
I tested it on my google spreadsheet integration here

As you can see on my shared spreadsheet, I added custom column at the right.
