I am now having an issue with getting my email notification. When I use the test button from your site it works fine, but when someone submits through my site, I never get the notifications. Please advise.

For testing purposes I appended my email address to your form to perform a test submission:
And I did recieved the email on my end:
I would suggest to try adding the domains used by jotform to your whitelist.

Or contact your email service provider to find out why the emails are being rejected. Alternatively, you can also try a different email address from another domain.
