I had to replace the Joomla script with the embedded code. Why did they stop working? I checked upon test and all was fine. an hour later or so none would display.

May I know if you're still experiencing this issue? I looked over your page http://snapoutofitwomensnetwork.com/index.php/tickets and could not find any problem with your form. It is working perfectly fine under Firefox, Chrome, IE, and Safari (Windows).
If your problem still persists, please let us know. We will be standing by, waiting for your response to this thread. Thank you!

Yes, they are working but they had stopped. I had to change the code form the Joomla Script to the embedded. I was a bad situation because we had just sent out a link for a special price on a webinar only to find the form wouldn't display. It did work an hour prior during the testing phase.
I then went to my other page with a Jot Form and it wouldn't display either.
I had to change code on both web pages to get them to work again.
That's my question... why did the original script that I pasted from clicking the Joomla button for code stop working?
But work fine now if I select the embedded code but will not display if I select the Joomla code.

Would it be possible to create a test page on your site and have the joomla code of your form embedded in it? This is so we can further inspect your site and find out if the cause of the problem is on our end or yours.
I tested the joomla code for your form in my own local test page and it worked perfectly fine. Most likely, the problem is on your site, but of course, we need to make sure.
Please respond to this message with the url of the test page as soon as you can. We are looking forward to helping you solve this problem.
