JotForm forms can definitely be set to be web responsive. Do note that the experience may vary depending on the phone, screen size and version of mobile browser being used.Â
I do not have iPhone handy. Nevertheless I have tested your site on a Nexus7 and a Android mobile device. And the form appears to load better when it is directly accessed from the Form URL as opposed to the website you have posted above. First suggestion I would make is to embedded the form utilizing the iFrame Method.Â
