Document Request vs a Form to show ownership and relationships

    We are a company that requires a lot of information on prospective clients. Hence we want to make it easy to provide to us details on their existing business. These client are private owners usually a family business with net worth well into the millions.

    We're looking for a way that they can tell us about the structure of their entity - a flowchart would be ideal, showing the ultimate ownership structure with relationships amoung entities....AND if family is involved - what is their relationship to each entity, For instance if there are two brothers and a sister that own an entity (or percentage of), then the family tree shoudl dtart with parents of those siblings and go down the lineage of each.

    Is anything liek this available through JotForm.

    There is a lot you can do with Jotform and it is sure that there would be something like what you are looking for in our Template Gallery

    You can search for categories so that it can be easier for you to find a form similar to the one you wish.

    Now it is easier to accomplish what you need with our List of Widgets, I think you can find useful widgets for your needs in the Survey category. One of the widget I think that may help you is our Matrix Dynamique widget that allow the user to add as many fields as they need to enter his data.
