Hi, I'm looking for a form that will only show adiditional fields if the individual checks a box I have within my form.
For Exampe, if this is my form:
Are you a Retailer?
 Yes, plese have a rep contact me.
**if they CHECK the box, then these fields show...(but don't show otherwise)

Can this be done with any of your current templates or be created from a black one?

You can do that through conditional logics, so that when a checkbox is ticked, it will show field one, field two, etc

Please check this guide in order to learn how to apply conditional logics
Be aware to add each field you want to hide through a New Action
You can also combine more rules, by adding a New Rule
