Multiple people sign up for Multiple Classes form exampleAsked by Debwork on March 04, 2014 at 04:25 PM
#1 I was wondering if I can add a field to attach an image to a form? I have a client who does shoe repair and seems it would be easier to answer some questions if asker could attach a picture. (How is it done?) I can't seem to find that in User Guide I figured that out
#2 Also... for someone else... Do you have example(s) of form(s) that would allow a family to sign up multiple kids for multiple different classes? I looked at your use cases but could not find. Mother signs up 3 kids for multiple classes.
#2a Then on the back end, since the classes change every few months, if a class is removed from the form - are there concerns about that with the database?
You can achieve this on the form using this guide
You can also search for a template on the JotForm Template gallery
If you search the gallery for class, it should suggest to you these templates
If you meant removing a field on the form -- removing field on the form with already existing submission data WILL DELETE the submission data of that particular field.
Hope I am able to answer your questions. Inform us if you need further assistance.
