I need assistance on a form with conditional logic -- NAEPE 2014 Registration

Under Wednesday, May 28, I have conditional logic set under "Please select your activity for Wednesday, May 28. I have redone this logic several times, but it will only accomodate 2 of the three selections. The ones for Golf Outing and Shooting Range display correctly, but when selecting "I prefer not to participate in any activities...I need the next three selections to  be hidden (the ones about reserving golf clubs, shooting experience, and shotgun experience.) The logic conditions are in there, but are not functioning. Every time I do it over...only two of the three will work. Can you help? (And I have not made changes in the form since doing the conditions...so that is not the culprit. Thanks! The form url is:

1. Remove/delete your current conditions associated to Please select your activity for Wednesday, May 28. field.

2. Apply this condition in your form.
