Alignment of Element out on Form HELP

I'm using jotform for the first time and I have just created the perfect form but I started having an issue with it this morning when the alignment of our elements went out. 

The "Instructions" and "Purchase Order" part should be next to eachother and they were until this morning. I can't figure out why Instructions have an indent? I have checked that the alignments are all left and this is the only field doing this so I don't know what is going on. It looks perfect in the jotform edit page - the problem comes in when trying to view it on website or jotform sample generator.
