I want to integrate a CTS (Cut To Size of timber panels) order and quotation form on my company's online cart. This form will be used by the user for requesting a quotation or placing an order online. The form will simply ask for (1)Board Type, (2)Thickness, (3) Number of panels, (4)Panel's height & width etc. The paper layout of the form can be seen in the attached pdf file.
As you can see in attached "cts.jpg" file there are 13 columns per row. I want to include only one row with these 13 coloumns in my web form. Now if a user wants multiple CTS panels he needs to fill multiple rows, for this purpose I intend to put a "ADD" button. Apart from this I also want to include a "Preview" button so the user can actually see the input data before submitting the request.
On exploring the JotForm online web form utility, I am very excited to use it as it seems very simple to use. I just want to know whether can I design such a form with the functionalities (by using ajax & jquery) mentioned above. If it is possible I would like to avail it.
Any help on this would be highly appreciated.

Unfortunately there is no such feature in JotForm the only options allowed
for a button is the Submit, Print Form and Reset.
You can probably use Matrix field instead to achieve something
similar to the sheet.
Matrix field can be found under the Survey Tools section:
The default input type is radio button you can however you can change this by
clicking on the Input Type button then choose Text Box:
Click the Rows and Columns button to edit the labels for the rows and columns

Can I get the HTML source code and the validation script for the web form to run from my own server instead. Somehow I don’t want to redirect any requests to external server(s). Please let me know how to go about it?
