I've took a look at your email address and its not in our bounce list. Did you just changed your embed code to form source code or was it like that since then? It is definitely a problem with your existing source, you have placed the Jotform form source code into another form tag which is the reason why the form is not submitting anymore. Re-embed your form using iframe embed code, this should resolve the problem. Guide: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
