    We have been using your services for months, but now for some reason we are not receiving submissions. Can someone please help me?

    Sorry to keep you waiting.

    I've took a look at your email address and its not in our bounce list. Mail log also shows that submissions was successfully submitted in your email address.

    Hi Teresa,

    Did you just changed your embed code to form source code or was it like that since then? It is definitely a problem with your existing source, you have placed the Jotform form source code into another form tag which is the reason why the form is not submitting anymore. Re-embed your form using iframe embed code, this should resolve the problem. Guide: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
