Why is the old version of our form showing on our email submissions.

We updated and renamed a form but when we receive the emailed submission it still shows the content from the old form. Here is a link to the new form http://www.jotform.com/form/11484138016.  Please advise how we can correct this.

I examined the form, looks like you either need to edit the existing email notificatin or create a new one (of which the latter is easier)
