    Why is my website URL showing up as pre-filled text in my form?

    The URL of the page my form is on is showing up in one of the text boxes. This appears when viewed in Chrome and on my iPad, but not with Firefox, in case that helps.

    Hello Eccssummercare,

    This is actually very strange. I was able to replicate the exact same problem you are having. There seems to be some conflict between form scripts and other scripts available of your page. Loading your form with its direct URL works just fine.

    I would suggest you to please embed your form using it iFrame embed code and that should solve your problem. The following guide should help you:  http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
