Issue printing signed forms

    Hi,

    I have created a form to collect personal information and receive consent for information release. This form is a clone of a clone of an original form I used for another project, but with significant changes in the header, consent wording, some of the information fields etc. When the new form is published it displays correctly for the new client, however when I download a pdf of the submitted version of the form it uses all of the text from the original form except for the header graphic, which is from the new form.

    This is in your HIPAA compliant section so I don't think you can dig in too deep. However, here are the two forms:

    I have attached downloaded pdfs of two dummy submissions so you can see the issue. I will add that in the cloned clone form, when the user reviews the completed form before submission it is correct. Feel free to fill either one out yourself if that helps.

    Thanks for providing additional details.

    Please open your form with incorrect PDF document in PDF editor by following this link below:

    https://hipaa.jotform.com/pdf-editor/212914641626152

    Then, you can follow these steps below and add a new PDF document that will automatically include all the corresponding form fields. Then you can also remove the previous PDF document with the wrong fields:

    After that, all the downloaded submissions should reflect the updated PDF document.
