What is your JotForm account username or the email address you used in your JotForm profile. I am unable to find any details with our email address "firstname.lastname@example.org" and that is the reason unable to check your account.
In Free accounts, you are entitled to receive up to 100 non secure, 10 SSL / secure and 10 payment submissions. If any of this limit is reached all your forms will be disabled. Most likely, you must have used SSL version of your form and you must have exceeded the allowed submission limits.
Do get back to us with your account username and we will take a look.

Thanks for providing the email of your account.
Please let me clarify that we have not changed the limits/benefits of the free or any other plan at all. They remain as are described in the Pricing page.
Please note that there are 3 types and limits of submissions: Regular submissions, SSL secure submissions and Payments. The assumption of my colleague was correct as you have reached the 10 SSL submissions limit as free user you have, however I have reset your counter to zero so that your forms cannot be disabled. We can do this action just once, so please follow the instructions given below.
That means that you have been using your form in secure mode (https://secure). To stop using it in secure mode make sure to disable it by unchecking the Secure form box in the Embed Form wizard:

SSL means Secure Socket Layer. It is a security technology used on the transfer of data through the internet. This makes it possible to securely transfer sensitive data from the users computer to your jotform account. Making one step harder for prying eyes (hackers) from snooping on the sensitive data being transmitted.
Generally SSL is used when it is neccesary to protect the data being transferred. The Standard version should be fine for more public affairs, when it is not neccesarily needed to overprotect data being transferred.
Do let us know if you need further assistance. Thank you.
