We purchased the monthly subscription to JotForm on 3/7/14 but our users are getting the below message. We submitted a request for help on 3/11 and have had no response. We have 200 people trying to use this form;
Additionally, our acount seems to have disappeared. We cannot login and we cannot request a password reset, the system does not recognize the email address we used to create the account.
The form did work correctly for about 20 uploads.Form over quotaThis form has passed it's allocated quota
and cannot be used at the moment.

First of all, let me inform you that your form is not hosted in a registered account, it is in a guest account. Creating your form in guest accounts is not recommended because you run the risk to lose the access to the form and to its data. That's why you cannot access to it anymore.
So, make sure to create one registered account here: http://www.jotform.com/signup
Then contact us again and ask us to move your form to the new account and share the Username with us.
Then we will be able to proceed investigating on this issue.
