Send Uploads as Attachment from Approvals email not working

I'm trying to send form uploads as attachments from the final email in an Approvals workflow, but the attachment is failing. The email comes through with a couple of attachments resembling HTML code, but the file uploaded to the form doesn't arrive. See the attached screenshot.

Can you please share which form it is so we can look at it and see if we can replicate this. Looking at the account for john.ross@nebraska.edu I don't see either of those forms having an approval flow setup.
