Hi, I've already done that and the link I provided previously shows the one I have done. My question should have been more specific, the PDF I have added has questions & multiple choice answers in it already, how do I use these questions/answers in jotform? When I load the pdf above with this info, jotform then asks me to type in questions/answers etc, I don't want to have to type them all in again, I want jotform to recognise the ones already in the pdf form.
How do I do that?

Hi,
If you want to prefill a form without typing in the answers all over again.
You can use the prefill feature and select 'Form prefill'

Hi, I've tried this (screenshot) and it still doesn't prepopulate anything from the PDF? I have the questionnaire in both word and pdf, I want to load it into jotform without having to type in every question and answer again.

Hi,
You can only pre-populate the form if the answers in your PDF document is from existing submission.
Jotform does not currently provide OCR feature.
You can only convert questions in your PDF: https://www.jotform.com/products/smart-pdf-forms/
