That's not really what I am looking for. Do you have an example of using the fill in the blanks to show it looking like the form from above?

Thanks for patiently waiting. Look like it's not possible to have two columns of fill in the blanks like in your screenshot. However, you can use it for the event type. See this demo form for reference.
