I would like to know how to retrieve a document that was sent through jot form

I would like to know how to retrieve a document that was sent through jot form. I saved the file however I cannot retrieve it for my records. Can you please help me with this situation. The receiver has received the form however I do not have record of the form on file

I just sent an email about my experience with jot forms. The screenshot I sent, the email on the form isn’t to whom I set the form to. I sent the form to VP Records if that may help retrieve the documents
