HIPAA account: Blank Submissions in Inbox and Table Views

    Several of the submissions for one of my client's HIPAA forms are blank in both the Inbox and Table views on the desktop version of jotform.com. This shouldn't be possible because the forms have several required fields, and this particular form is only accessible via redirects that autofill certain fields. I can also edit these submissions and see the customer's filled out information. My client has noticed that data is visible on the iPhone app as well.

    This is causing major workflow issues at one of my client's pharmacies, who is offering COVID-19 vaccinations and boosters. They rely on the Inbox and Table views for knowing when customers have booked appointments.

    I believe the form's ID is 210175904390048. The direct link is not public. A consent and information form is required first, then customers are redirected to a form solely for booking appointments. Here's the consent form:
