Our form has been working fine for months. Today only a portion of it loads. Can you please look into this? at www.txt-book.com click on "iPhone Service" in order to load the form. Thanks in advance.Page URL:

It's loading fine here. But anyway, I cleared your forms cache. I hope this helps. One thing I noticed is that, your form doesn't have a submit button. Is that intentional? Please add one if you want your user/customer to submit the form.

Hi, the problem is that the form you are seeing is only half the form. The change log refelcts many items removed (12) -- we have not made any of these changes. The reason their is no submit button is because it was removed.
Revisions instances from february 28th and older do not load -- they are just code. Can you please look into this -- we need to restore the form to its function version and also stop whatever it is that is making these unauthorized changes. Our team is two people -- nobody else has access to the form.

We do apologize for the inconvenience this may have given you. It appears that the form is displaying as designed although I am not sure as to what could have caused removal of any fields.
In any case moving on I would suggest to have a clone of your form whenever you havecompleted final changes to it.
At this time I will raise a ticket to the development team in regards to the code shown for the revision history from Feb 28th and earlier. Once we have any updates from them, you should be notified through this thread.

Hi Cesar - It looks like another user just posted about missing fields as well... I would wager there is something larger occuring. I don't want to rebuild the form if the underlying issue is not resolved.
The clones in the archived folder from months ago also do not contain the missing fields. They've disappered in the exact same place.
Can you please look into this further?

Yesterday, one of the servers had a full hard disk. That probably caused the problem. I have now reversed the form the way it was using the revision history. If the problem continues please let us know.
