    Form had been tampered with.Questions have been removed and only logo remains.

    I send this form to all my customers and it is also on my website www.Motodojo.com. I used this form yesterday with no problems and today it is only a image of my logo, the entire form is gone. I'm the only one to edit this and have not touched it side I created it so this must have been done ppb jotforms end. please help with this asap.

    Same thing has happened with my form today.  And there is a history of recent changes that I didn't make.  Something has to be going on since the same thing happened to you.

    It appears both have a similar issue,. yet they both still appear to be rare instances as a large scale event/trend has not been identified. In any case, I will escalate this matter to the development team so they are aware of this incident. This way they can further evaluate which steps to take. Once there is any update from their end, they will let us know through this thread. Thank you

    Ted: One of the servers had a full hard disk on sunday. That might have caused this problem. I have now reverted your form back to the way it was using the Revision History feature

    txtbookapp: Same problem. The revision history saved the form.
