I'm not exactly sure why your form is indexed by Google. Let me forward this to our higher ups so they can review and provide us insights about this.

We do not give google list of forms or anything like that. The form IDs are also almost impossible to guess. It must be linked from a public page. That's the only way google can find and index it.
To find out where it is linked from you can try adding google analytics to your form:
If you do not want it to be listed, clone this form, start using the new one and then disable the old form.
