Hello,
I have created a form and once i submit the form, i am not seeing the full report in Google Dive/ integration, only part of the from?

Happy to help!
Could you kindly disconnect and then reconnect your Google Drive integration for the form in question and then try with a test submission? I have gone ahead and cleared your form caches as well.
Please let us know if the issue persists.

Thank you for letting us know!
I created a clone of your form 213005651413038, integrated to Google Drive, and sent through a test submission but was able to see the form properly in Drive.
If possible, could you kindly share a link to the submission in your Google Drive so that we may view it and access it should we need to escalate this thread?

Could you temporarily set your sharing settings for that link to Public for now?

Thank you for the access!
It looks like perhaps the PDF itself has not been updated or is not complete: indeed, in the clone of your form I made, it looks the same as the PDF in the link you sent me.
So, if you create a New PDF, you should be able to see that it includes all form fields:
Then, you can delete the old PDF so that this new, longer PDF will be the PDF sent through the Google Drive.
Could you please try this and then submit another test submission to see if the PDF is corrected in Drive?
