I use to get the whole document now I am only getting half of the information in the email the employee says they filled out everything on the jotform and I do not get the whole jotform.
Important information that is needed as Wisely card # a asset tag number for returning technology and the calendar is no longer showing on the email or the attachment. Pleas show me how to correct.

Hi there,
Can you show us what your emails used to look like and what they are now? If you can share a screenshot, that'll be better. You can follow this guide for details: How to Post Screenshots to Our Support Forum.
Currently, your form's email notification is set up to show up to this part only.
One of the things that I can see is, that the last option doesn't have the Form Field specified. This will cause the email to show it as blank even if there's data submitted there.
If you can share with us the kind of information that you're looking for from the email, then we can see what went wrong with the notification.

Hi,
Thank you for the attached images.
I have cleared your form cache. May I ask you to create a new Notification email on the form, and make a test submission?
You can use this guide to create the email:
Setting-up-email-notifications
Please see if all of the fields would be visible on this new email. If the new email works, you can customize it and adjust the conditions to it.
