To Whom It May Concern:
I am trying to make a simple form (called, "Playdate Form") and I need to be able to put data entered from one field into the answer choice options in another field.
Please tell me how I can do that?

Just to make sure I got your query correctly, do you want to autofill a single or multiple choice option based on the data entered on a particular field, is that correct?
Please check the screencast below, and see if that's what you're trying to accomplish.
If so, then that is possible using the Insert/Calculate field condition.

No, that is not what I am trying to do as there will ultimately be hundreds of different variables. I created a sample form called, "Playdate Form" please look at that as an example.

Hi there,
So, I've tried to figure how to achieve what you've asked for using only the tools that we have available (More than that would be to use HTML and JS)
I couldn't yet manage to get the Radio buttons to work(Name 1 or Name 2)
but I did manage to get the tick boxes working using the field calculator and empty tick boxes
Although it's not the prettiest, that could be easily designed using CSS.
