    Regards,

I'm Fiverr freelancer and design multiple Jotforms on daily basis. Here's my fiverr username "aqsablogwriter". My jotform ID is aqsaakhtar148@gmail.com

Recently I designed 3 forms for my client related to Wallet Support and Jotform automatic system has banned my IP Address,

    Form 1.

    https://form.jotform.com/walletsupport/metamask-wallet

    Form 2.

    https://form.jotform.com/walletsupport/exodus-wallet

    Form 3.

    https://form.jotform.com/walletsupport/trust-wallet

I requested review but didn't received any response yet. You can check, that all forms are genuine and don't involve any fishing activity. Now, I can't design Jotforms due to this restriction while my other clients need it. Please look into this matter and activate my IP Address and also my clients Jotform account. His username is "WalletSupport".

Right now, I'm using VPN to make review request. My actual IP Address is 192.168.100.1

    and 192.168.100.2

    Regards

The forms that are found under your account (WalletSupport) were flagged as scam for it offers fake services and uses copyrighted logos.

We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

In order to reactivate your account, we will have to delete the offending form.

Or, kindly provide us the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

Send these proofs to reviewers@jotform.com with Thread #3482168 as your subject. We will let you know about the status of your account after a thorough review of your proofs.
