Could I please request that you can provide some screenshots with regards to this particular form? It would appear to be working fine from the side. A couple of screenshots would provide greater clarity so we can suggest the best possible solution or escalate it to the appropriate team to resolve for you.

Dear customer,
Thank you for your reply.
Please select the form you are referring to and click the button 'Open in builder'. From there please click the form builder button on the top left hand side and then click on the PDF editor. After that select new PDF document which will contain all the fields that you have in your form.
Once you have completed this please refer back to your builder and select settings. From there click on the tab 'Emails' on the left hand side and go in to your emails wherefrom you will need to select 'Advanced' and finally update the PDF.
