    My secure forms are losing their style and formatting and some elements are not loading -did something change recently?

    I have 2 active forms that were working fine on Friday.  However, we went back today to use this form today and one of the forms is missing the submit button the other form is missing several of the last pages.  I did NOT do any updates or changes to these forms over the weekend.  Did something happen (site maintenence) over the weekend that would cause this??  Thank you for any help you can provide.

    I also  noticed that all of my conditional settings are gone from one of these forms as well!  Any way to get all of this back?  This was hours of work that need to get up and running again ASAP.  Again, thank you for any help you can give me!

    The website is http://music.hilliardschools.org/tharp/band/directors-only-band-evaluations . It is a password protected page and the password is "bandplacement".
    All of the forms in my account were altered over the weekend. I was able to fix the top link "woodwinds/brass" by adding the submit button back and embedding the code again. However, the last 4 pages of the "percussion" form are gone. All of the data from those pages are gone as well in the submissions.
    In honesty, I am quite nervous as that is important information that I cannot get back again. So anything you can do to help solve this issue is greatly appreciated. Thank you!

    Hi Nicole,

    Still, the page returns a 403 Access Forbidden Error. Can you please re-check?

    It's impossible for someone to access your account, not unless you share it to your colleagues. We also don't modify user forms, not unless requested. 

    Can you also please check your form revision history: Introducing-Form-Revisions-Never-Lose-Your-Work-Again? Check all the revisions, and if you see a revision that contains the lost pages, you can revert to it, and you should be able to retrieve all the submissions as well.

    Thank you for your continued help with this. I removed the password protection, as maybe that is the reason you are unable to view it. Here is the link again: http://music.hilliardschools.org/tharp/band/directors-only-band-evaluations It is the "percussion" link that is the problem.
    I did as you suggested with the revision history. I can see where all of the stuff got deleted (I still have NO idea how it was deleted because I did not delete it and no one else has access to it), however, it will not let me revert to the version prior to these deletions. This is the response it gives me for every form prior to the problem:

    If I go and rebuild my form (add in all of the form fields and widgets), will I get the data from the previous submissions from these missing fields back?

    I too am having this issue.  If I remove the "secure" option in the Embed Form section, the form looks fine.  Change it back to secure and it is messed up again.

    We are also seeing a problem with secure forms.  We have determined that many of the web page elements are not loading properly.  Using FireBug (see below), we are seeing 404 Not Found from elements hosted at https://static-interlogyllc.netdna-ssl.com .  It shows I am accessing the Seattle datacenter when I browse that URL.  Could it be that the NetDNA servers JotForm is hosting content on are not updating at all of their datacenters properly?  One of our forms is at https://secure.jotformpro.com/form/31706216535955

    There was an issue with our CDN on delivering content to secure forms in some locations.

    We have temporarily reverted the cause, so your forms should be fine now,  but we have also asked our CDN provider to fix the issue ASAP.

    Thank you for your patience.

    It would also help if you were to open new support requests if the issue persists.

    Please let us know if the issue persists.
