Unable to View Full Form in Submission with Responses

Hi,

I need to be able to view the results of this form with the results appearing along with the full question/data that was asked.

I can't seem to load it in PDF Editor. If you notice the two screenshots, one of the signatures is supposed to go along with the other image. But the submision doesn't show both at the same time.
