Order form without payment options

I'm looking to create an order form where the user will only submit a PO number. They will not submit any kind of payment. I can't seem to find an order form template without payment info or a way to delete the payment info within a form. Appreciate your advice.

Do you want a text field only for the PO number and not the complete Product List widget? If yes, then you only need to click the payment widget and then click the Trash icon (as shown below):

Thank you Patrick. I still need the Product List widget. But if you look down at the very bottom, it has "Payment Options" with "debit/credit" and "paypal options" listed underneath. I want to do away with all of the payment options because the customer will not pay with the placement of their order, they are billed later. But there is no trash can on the payment options. Thanks so much for your help.
