On the following form that I have created https://secure.jotformpro.com/iconicperformances/Feedback-Survey-Review-Form
When clicking on Review Educational Show, it successfully takes you to the appropriate URL https://secure.jotformpro.com/iconicperformances/EducationalShow-Review , however no matter what you select it produces the following error message "There are errors on this page. Please fix them before continuing."I am not sure why, as it use to work before.

Upon checking your form, I found that you have a injected following custom css to hide form field "(NO LONGER ACTIVE) Date of Event:":
This field is actually a required field and that is the reason you see the required validation error message on your form.
To solve this problem, you should mark this field as non mandatory as you are anyway hiding it from the form. Another way to solve your problem is to deleted the above injected css and click on "Hide Field" property of your field. Please check the screenshot below:
