Any idea why our form is suddenly garbled?

    We are using a templated form, located here: https://www.finstaff.com/application/.

     

    For some reason, it suddently is all garbled.  It should look like this:  

    http://form.jotform.me/form/40224211556443?

     

    I have no idea what happened...  It is a secured form, and all parts of the code have "https"...  Again, it was working, but suddenly it is not working.  It appears like this:

    We are terribly sorry about this inconvenience, our developers are very aware of the problem and are working to have this repaired ASAP

    MaxCDN (the CDN we use to serve your forms faster) was down in some regions and we disabled it 5 hours ago. That fixed the problem.
