How can I fix it or create a successful form

    My page seems to no longer exist while a customer successfully completed the form and can no longer open the survey

    Hi there,

    Can you give us a link or the title of the form that you're talking about?

    I just checked your account and I don't see any deleted forms recently. And you still have 1 active form. So it should still be accessible if that's the form that you're talking about.
