My forms are all gone?

I created a few forms back in September to try it out and when i logged in today it showed that all of my forms are gone. If i click submission links i received in my email i can view the results of any given form, but i cant view the form itself or see the rest of them.

I need to grab some information from the questions i had in there. is there any way to recover them? thank you
