How to prepopulate field on one form at the end of the form prior to submission.

I have a form that asks for birthdate at the beginning of the form. I want the birth date to reappear in the same form towards the end on the consent page that the patient has to sign on intake. This is prior to submission.

I do this for name and it works and for address and it works using the conditions, if then statement features under settings.

This doesn't work for birthdate, what the heck!
