Page cannot be found for the page: http://www.vangovans.co.uk/reservation. I checked your forms, but I couldn't find any Notification Email to both. Did you remove the settings?
Please add it again: Setting Up Email Notifications then make sure that Sender Name and Email is configured correctly.
