    Why does my url automatically show's up in the first name field - my form users need to delete the url out of the 1st field to enter their information

    Can you please try re-embedding the form again to your google website. 

    On initial check using the form's direct URL https://www.jotform.net/form/40574988153263  does not show similar anomaly.

    When you re-embed, please make sure that the embed URL does not uses any URL parameter value that can prefil the "Parent's Name" (my hunch is, that could be the only possible reason why that field have value on it already)

    Hi. The same thing happened to me before. In my case, what happened was I accidentally placed a text on the Default Value field. 

    1. Select the textbox (Parent's Name: )

    2. Under Properties, check Default Value

    Chances are you might have pasted, or what not, your site's url on the box. I hope this helps you.

    I re-embedded the form again to google sites by following the instructions in the embed menu on jotform.  The problem continues. My coding experience isn't extensive, but from what you've mentioned I don't believe the embed url includes any url parameter values. Can I post the code here so you can take a look? any forseeable security concerns if I do?

    Thanks njrefine, I checked the default value, it's all fine.  I even inserted a single space as the default value, hoping it would overwrite the url that keeps appearing, but the opposite occures (url overwrites the space during each refresh).

    Have you tried already using iFrame embedding method? If not, can you please try it to fix the issue?

    Thanks pinoytech!  I copied and pasted the code, now everything is working perfect.  I appreicate your help.  I had tried iFrame in the past but must not have been coding it correctly.  I'm not sure what you did differently but the important thing is that it's working now. Thanks again.
