First of all after spending hours and hours last night researching forms to use on my clients website i came across JotForm and by far this is the best!!! I just LOVE IT!!
My client is looking to host the form themselves. I understand we can download the source code (thank you!!) but when i change the action="http://www.jotform.com/submit.php" to the one hosted on my clients server the captcha stops working and when a required field has not been filled out the http://www.jotform.com/images/exclamation-octagon.png image is missing.

Captcha API is website-specific, meaning you cannot use an API meant for our site on your own domain. Please click here to get your own Captcha API and implement it on your site.
With regards to the exclamation image, it is working fine on my end. I am able to see this exact image: http://www.jotform.com/images/exclamation-octagon.png. It could be your browser or your internet connection that's causing the image to not appear.
Try clearing your browser's cache or resetting your internet connection.
