Search for previous submission from within the form

Hello

We have a call log that our receptionists use to log all incoming calls. Is there a way to add a "Call History" button that would show previous calls made by the current caller once their name is entered. Right now, I have given them "view" permissions to the Google Sheet so they can search it when a caller has called before but it would be fantastic if that could be done within the form.
