Personalize my jotform links

I would like to have my health assessment jotform link more personalized

right now it is https://form.jotform.com/212433519141144

what I want it to be is http://form.jotform.com/micheleknisley.health-assessment

Same thing with my coach explore jotform link

right now it is http://form.jotform.com/212444205271142

what I want is http://form.jotform.com/micheleknisley.coach-explore

What's happening when you click on the pencil icon to edit? When I click on it I can change what it is and then hit apply.

When I click the pencil it let's me remove part of the link only so far then it stops

That's because you can only personalize your link after your username. Example:

https://jotform.com/Wensel_michele/{CustomizeMe}

If you wish Jotform not to appear on your form links, you can embed your forms on your website so that only your website URL will show to your users.

Otherwise, you'd need to upgrade to Enterprise to fully customize your form's URL.
