Is there a way to create video thumbnails with radio button for selection?

I would like to place videos on my form and have the user selct which one they prefer.

Is this possible?

Hi Mark,

You can embed videos and make it as your radio button options, but I'm afraid the result would not come as the video. So, for this to work and so you know the selected video, I would advise to add the video title. Look at this example form: http://www.jotformpro.com/form/40795688797986
