Please remove CAPTCHA from my form

Hi,

I just created a form called MID v6.0 in my account, and a CAPTCHA test was included after form submission. From searching on the forum, I understand that you may need to remove this for me. Could you please do that, or let me know what I can do on my end to remove it myself?
