Lost user name and password to account

I can't find user name and password to account. I have listed the form URL below, this is the one I want to change. How can I get this info so I can make changes? Thanks.

I don't want to have to create a new account so to make the changes by creating a new form.

We would like to apologize for any inconvenience, however, we don't have any information with regards to the password of the account. 

I have checked the website and the form embedded in it belongs to an account that has the username, betterlife2b.

You can use the username to retrieve the account's password. 

Just go to Jotform's login page and click the "Forgot Password?" link. Enter the username on the "Remember your Username or E-mail" field and click the "Send Reset Instruction" button. An email will be sent to the email on the account with the instruction to reset the password.
