Sheet to form with multiple choices

Hi,

I want the parents to put in their email address and verify it using Sheet to Form option, which would automatically populate their child's name and a class. The problem I have is similar to vlookup, where if there are multiple children attached to the same email address the form does not actually pulls this information out, and only populates the class of the first child and not the name.

How can I set it up so that if a parent puts in their email, they have a choice (perhaps another way is to use the dropdown menu?) to have a choice to select only one or the other of their children, without seeing the rest of the students.

Hello Vlad,

We're sorry, but it's not possible. The workaround would be to have a new row in the source Excel file for each child, and each with a unique identifier for the parents to pull through the spreadsheet to form widget.
