If you are the form owner, kindly go to your account settings and uncheck "Require log-in to view uploaded files".

However, if you only filled the form, I suggest that you contact the form owner regarding this. If you have received a confirmation email after you submitted the form, then you can use the Reply-to option on that email to get in contact directly with the form owner.
