Which template would you recommend for a typical petition to present to city council?

    I'm using the latest version of WordPress.

    I need a template for people to "sign" with the results presented to our city council as a simple list.

    The required fields are:

    • Static text field for instructions with embedded URL link
    • Static text for the petition "ask"
    • Input fields:
    • Name
    • Property address
    • email address
    • Ward (drop-down selection list with 8 text entries, or text)
    • One or more additional text fields
    • Submit & Cancel buttons

    After a submittal, I would like to display a confirmation Wordpress webpage.

    Of course, I need to capture the input, ideally as an Excel workbook or text file of delimiter-separated values.

    I could possibly deal with emails sent for each submit, but that wouldn't be my preferred approach.

    Hello Paul - Let me answer each one by one. Please see my responses in blue:

    • Static text field for instructions with embedded URL link (You can use a Paragraph field for this. Hyperlinks are allowed to be added.)
    • Static text for the petition "ask" (I guess you can also use the paragraph field for this one, unless there's an input required.)
    • Input fields: (we have the basic fields you can use for these)
    • Name
    • Property address
    • email address
    • Ward (drop-down selection list with 8 text entries, or text)
    • One or more additional text fields
    • Submit & Cancel buttons (We only have a SUBMIT button. There's no CANCEL button unfortunately. Why do you need one? The user can just skip answering or filling out the form if they don't want to.)

    You can redirect the user to your own Wordpress page instead of showing them a default Jotform Thank You Page. Here's a guide on https://www.jotform.com/help/38-redirecting-users-to-a-different-page/
