Would this be a good app to use to have people sign consent forms as well as fill out simple medical history forms? It needs to be hippa compliant. Are you able to use a send and sign? And where are forms stored after they are filled out?

Jotform is an online form builder. We provide a lot of tools and 10,000+ customized templates. It's very easy to create forms thanks to dragging and dropping. To do this, please follow these steps:
- Click My Forms on the top menu.
- Click the Create Form button.
- Select the option you wish.

There are several methods to reach the collected data. First of all, they can be viewed on Jotform in views of tables, inbox, and PDF reports like below. Secondly, the data can be downloaded by the form owner. Also, all submissions are emailed to the email address of the form owner as default and it can be customized.
