Spreadsheet to Form widget: Fields is not auto populating

I put excel sheet but no options are filled in human resources dropdown.

Apologies for any delays. Upon checking further, I found that the issue is with the drop-down field on your form. It appears that there are still no options added under it.

Please try to add options on the drop-down field on your form that matches the data under the Human Resource column of the Excel File you uploaded on the Spreadsheet to Form widget.

is it possible the options pre populate from excel sheet?

I'm afraid that it is not possible to get the options of the fields on your form from an outside source. You would have to set the options on a dropdown field on the builder page. Otherwise, you can change it into a different field type, like the Short Text element.
